Ground rules

How to pick your survey questions

Every template below was written for a specific audience and goal. Before you pick one, five rules will save you a rewrite later.

  • Write for one audience. A question set aimed at everyone fits no one, so send each group its own survey. Weak: asking staff and customers about "your experience with us" in one form. Better: one employee survey and one customer survey, each sent to the right list.
  • Cap it at 10 to 15 questions. Completion rates fall as surveys grow, and the people who quit first are usually your busiest respondents. If an answer will not change a decision you plan to make, cut the question.
  • Keep one scale throughout. Switching between 1-5 stars, 0-10 scores, and agree/disagree mid-survey produces answers you cannot compare. Pick one rating scale and hold it to the end.
  • Ask one thing at a time. Double-barreled questions make every answer ambiguous. Weak: "Was the training useful and well organized?" Better: "Was the training useful?" followed by "Was the training well organized?"
  • Keep the wording neutral. Loaded words push scores up and hide real problems. Weak: "How much did you enjoy our award-winning support?" Better: "How would you rate the support you received?"

  • How satisfied are you with the app's login and authentication process?

Feedback

For collecting reactions right after an event, meeting, or presentation, while the details are still fresh.

  • Was the content well-structured and easy to follow?
  • Were the topics discussed relevant to you?
  • Was there adequate time for each agenda item?

Healthcare

For health, wellness, and lifestyle research where respondents report their own habits and symptoms.

How many questions should a survey have?

Aim for 10 to 15 questions, which most people finish in under five minutes. A quick pulse check can be 3 to 5 questions, and in-depth research can stretch to about 25 if you group questions into labeled sections. Past that point, completion rates drop sharply.

Should I use open-ended or closed questions?

Mostly closed. Rating scales and multiple choice are faster to answer and easy to compare across respondents. Add one or two open questions at the end, such as "What one thing should we improve?", to catch problems your closed questions did not anticipate.

How do I write unbiased survey questions?

Use neutral wording, ask one thing per question, and give balanced answer options, including a way out such as "not applicable" or "prefer not to say". Watch for hidden assumptions: "How often do you exercise?" needs a "never" option to stay honest.
