OCT reveals calcium fracturing with multiple non-flow-limiting dissections after Wadge NC scoring balloon in heavily calcified coronary lesion. No coronary perforation, slow flow, or thrombus formation was observed. The dissections were classified as non-flow-limiting, indicating they could be safely monitored or covered with stenting if further expansion was required. Based on these results, the intervention proceeded with DES implantation, followed by high-pressure post-dilatation to ensure optimal stent apposition. Repeat OCT confirmed excellent stent expansion and full apposition.This case highlights the utility of Wadge NC scoring balloons for plaque modification in heavily calcified lesions, demonstrating how OCT guidance can confirm effective lesion preparation and guide safe stent deployment. It underscores the importance of controlled calcium fracture to achieve optimal PCI outcomes while minimizing complications.
