How do you currently manage patients taking buprenorphine chronically in the perioperative period for moderately or very painful surgery (orthopedic surgery, for example)?
I stop buprenorphine about 3 days prior to surgery and restart it once the postoperative pain has subsided.
I continue buprenorphine throughout the perioperative period and manage analgesia primarily with non-opioid analgesics.
I continue buprenorphine throughout the perioperative period and use opioids in a similar fashion to what I typically do.
It depends on the reason the patient is taking the drug and the risk of relapse if it is being used for addiction.
I do not encounter patients taking buprenorphine undergoing surgery in my practice.
